I understand that yoga includes physical movements which can result in injury. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. I hereby waive my right to sue XXXXX and all of its teachers for any injury, illness, or death if it should occur as a result of yoga practice in our facility or any other place.

Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. By signing, I affirm that I am in good health and physical condition to participate in yoga classes at XXXXX. If I am pregnant, become pregnant, or if I am post-natal or post-surgery, my signature verifies that I have a licensed medical practitioner’s approval to participate in yoga classes at XXXXX [and in relation to its entire content online]. I hereby agree to irrevocably release and waive any and all claims that I have now or may have in the future against XXXXX and its Teachers. I have read and fully understand and agree to the above terms of this Liability Waiver Agreement. I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional release of all liability to the greatest extent allowed by law in [the State of XXXX].
